Hospice Travel Social Worker

About the role

Agrace, a nationally recognized nonprofit leader in hospice and palliative care, is seeking a Travel Social Worker to provide patient‑ and family‑centered social work services across new and existing market areas. This part‑time position (20 hours/week) involves on‑site visits, enrollment support, and coordination of care for individuals entering hospice or palliative programs.

Key responsibilities

• Travel within Agrace’s service region to meet patients and families, conduct psychosocial assessments, and develop individualized care plans.
• Facilitate enrollment by answering questions, overcoming objections, obtaining informed consent, and completing all required documentation.
• Prepare and submit insurance paperwork, DME orders, pharmacy enrollment, and other referral documents.
• Collaborate with discharge planners, hospital staff, and internal teams to ensure smooth transitions from referral to admission.
• Provide scheduled and unscheduled informational visits, tours, and education to prospective patients and families.
• Support the Admissions team with follow‑up, documentation flow, and training of internal and external partners as needed.

Required profile

• Licensed Social Worker (LCSW or equivalent) with experience in hospice or palliative care.
• Strong interpersonal and communication skills to engage patients, families, and healthcare partners.
• Ability to travel frequently within the designated geographic area.
• Commitment to patient‑centered, compassionate care aligned with Agrace’s philosophy.

Required skills

• Proficiency with Electronic Health Records (EHR) documentation.
